Payment Information
The conference registration fee is $150 per person (plus $5 service fee if paying by credit card). This includes continental breakfast and lunch on October 2-3 as well as a copy of the Healthy Foods, Healthy Moves Replication Manual. Please indicate payment method below:

Lodging Information
Participants are responsible for making their own hotel arrangements. A discounted block of rooms is available at the Union League Club of Chicago. Please call them directly at 312-427-7800 to make your reservation. Please ask for the “Healthy Foods Healthy Moves” group rate of $190 (plus tax) when you call. Please book early as the room block may sell out.
 

Cancellation and Refund Policy
Conference cancellations must be received in writing by September 22, 2008 and will be subject to a 20% cancellation fee. Cancellations received September 23 through October 1, 2008 will be subject to an 80% cancellation fee. No refunds will be given after October 2, 2008.
