The Healthy Foods, Healthy Moves: InForm Chicago (HFHM) program educates children ages 3 to 18 on the importance of healthy eating and physical activity.

The Consortium to Lower Obesity in Chicago Children (CLOCC) and Communities In Schools of Chicago (CISC) have worked together to develop the program components, which include training teens, school staff and neighborhood program providers to be nutrition and physical activity message ambassadors in the city’s schools and communities. They all share a common message 5-4-3-2-1 Go!™.
